If you have already initiated a formal information request process with a government body, before we can fully analyze the merits of your case, we will need to review copies of all correspondence between you and the agency. (See transparency intake memo)

When you click the “submit” button after you complete this questionnaire, we will automatically email you a copy of your content. The reference identification number or term you create at the top of this form should be used when communicating with us about your case.

In addition to completing this form, please mail (do not fax) copies of your correspondence with the agency to: Western Environmental Law Center, 1216 Lincoln Street, Eugene, OR 97401. Clearly note on the outside of the envelope: “Attention: Transparency Initiative.” Please do not forget to provide the reference identification number or term you created at the top of this form with these materials.
